Donations & Registration For More Than 1 Participant...

If you are registering more than 1 participant for an upcoming Daily Health Observation Training event, provide the first & last name of each person you are registering.

Then, consider the TOTAL amount you are able to donate for the group and indicate that amount on the form.

Lastly, be sure to check the EVENT DATE you are registering your group for - there is currently only one available date in April, so simply check the box!

Submit form and an invoice will be sent to the email you provided so that you can make your donation.

Once your donation has been processed, a link to register for the workshop will be emailed to you.

Your group IS NOT considered registered until your invoice has been processed. This training fills up quickly & UNPAID INVOICES WILL CANCEL WITHIN 24 HOURS & YOUR GROUP MAY BE ADDED TO A WAIT LIST, so we suggest making your donation IMMEDIATELY UPON RECIEPT of your invoice. In addition, any person participating together in a group must have camera on and be in view throughout the training workshop. You can donate & register groups up to 20 people, but no more than 5 people can participate from the same shared device AND ALL GROUP MEMBERS MUST BE IN VIEW OF CAMERA WITH CAMERA ON. Groups larger than 20 are encouraged to submit a contact form to set up a private DHO training event.
